REFERRING PROVIDERS

We look forward to collaborating with you and appreciate the care and attention you provide to patients with all forms of eating disorders. The following referral forms are required to be completed by you to ensure we have the necessary information to proceed with insurance authorizations and admission to our IOP.

ADMITTING CLIENTS

Whether you are stepping down from a higher level of care, or making a step up to receive more support than office visits, we are honored that you have chosen our program for this phase of recovery. Our team will make every effort to ensure that you receive the highest standard of care and will utilize every opportunity to ensure your insurance company authorizes you to participate.

When you arrive for your first day of IOP, you'll meet with one of our clinicians for orientation to the program. There, you'll be required to fill out the following forms. To expedite this process, please print these forms out and bring them with you to your orientation.

LODGING

 For clients considering one of Chrysalis Center's Intensive Outpatient Programs (IOP) who live outside of the Wilmington area, we have partnered with Home2 Suites by Hilton to provide a comfortable and affordable place to stay during treatment.

Each of Home2's studio suites feature a kitchenette with a refrigerator, microwave, and sink - an especially helpful feature for clients in our Eating Disorder IOP, who may need space to prepare and store meals as part of their recovery. Guests of Home2 also enjoy complimentary breakfast, parking, and Wi-Fi, making it a convenient option for those seeking short-term housing while attending IOP. This lodging option is available to clients in both our Eating Disorder IOP and Trauma-Focused IOP, with exclusive discounted rates offered through Chrysalis Center.
